Enrollment in a Second Chance DUI Program is a four step process. The Payment Agreement is Step Three.
Do not proceed unless your court or DMV papers have been approved by Second Chance DUI Program in Step One: ENROLL DUI PROGRAM
And you have signed the DUI Program Agreement in Step Two: ENROLLMENT AGREEMENT
STEP ONE: UPLOAD DOCUMENTS
Upload Your Document in the Upload Documents Form to the right. You will need ONE of the following documents:
- Current Court Paperwork. No more than 30 days old.
- K4 Print out from DMV. No more than 30 days old.
- DMV Hearing paperwork.
Second Chance DUI staff will review and approve your documents. Once approved, you will receive an email to the email address you provided when uploading documents. The email will include a LINK to the Enrollment Portal.
STEP TWO: SIGN THE ENROLLMENT AGREEMENT
In step two, you signed the DUI Program agreement electronically. You were automatically transferred to this page to sign the Payment Agreement.
STEP THREE: SIGN THE PAYMENT AGREEMENT ⬅️ YOU ARE HERE
Sign the Payment Agreement by selecting the correct DUI Program in the buttons to the right.
Once that agreement is signed, you will be automatically transferred to a page for Additional Documents.
STEP FOUR: SIGN ADDITIONAL DOCUMENTS
On this page, you will:
- Download the MADD flyer, available in English and Spanish.
- Select the Demographics Questionnaire and follow the instructions for completing the form.
- Download the DMV document: DL-804.
- Upload the completed form in the Payment Portal
You will be automatically transferred to the Payment Portal to complete your registration.
STEP FIVE: MAKE YOUR PAYMENT
Once all the documents are electronically signed, you will be automatically transferred to the Payment Portal. Select your payment and make the payment using a credit card.
Sample Payment Agreement
Download a sample payment agreement. Read through to ensure understanding and agreement the payment schedule based on the selected DUI Program.
Select the DUI Program you are enrolled in to complete the Payment Agreement. If you are paying for the program in full and do not need a payment agreement, select Pay in Full. After the payment agreement is signed you will be automatically transferred to a page to sign the final documents required. You are almost done!
