- Electronically signed the DUI Program Enrollment Agreement.
- Electronically signed the Payment Agreement or passed to pay in full.

Download the MADD Flyer
You do not need to complete the MADD program before you take and complete your DUI Program. However, The MADD program is required to complete your DUI Program. You can take the MADD course anytime that is convenient to you.
The flyer contains all the information you need to enroll and complete the MADD program. The flyer is available in English and Spanish below.

The Enrollment Process
The enrollment process is a five step process. You are on step four!
STEP ONE: UPLOAD DOCUMENTS
Upload Your Document in the Upload Documents Form to the right. You will need ONE of the following documents:
- Current Court Paperwork. No more than 30 days old.
- K4 Print out from DMV. No more than 30 days old.
- DMV Hearing paperwork.
Second Chance DUI staff will review and approve your documents. Once approved, you will receive an email to the email address you provided when uploading documents. The email will include a LINK to the Enrollment Portal.
STEP TWO: SIGN THE ENROLLMENT AGREEMENT
In Step Two, you signed the DUI Program Agreement.
STEP THREE: SIGN THE PAYMENT AGREEMENT
In Step Three, you signed the DUI Program Payment Agreement or skipped the step because you will pay in full.
STEP FOUR: SIGN ADDITIONAL DOCUMENTS [YOU ARE HERE]
On this page, you will:
- Download the MADD flyer, available in English and Spanish.
- Electronically sign three documents in one step:
- DMV DL-804
- Text Notification Agreement
Once signed, you will be automatically transferred to the payment portal to make your initial or full DUI Program payment.
STEP FIVE: MAKE YOUR PAYMENT
Once the Payment Agreement, Remote Client Services Policy and the Text Notification Agreement are completed, you will be automatically transferred to the Payment Portal. Select your payment and make the payment using a credit card.
